We are posting this notice on our website because Liv-On Family Care Center, PA is committed to our patients’ privacy.  We want to make you aware of a situation that occurred at our clinic on October 25, 2020.  On that day, we discovered our clinic was robbed and we had stolen computers, laptops, and tablets.  Although all our devices were password protected and we have no indication that data was breached, the devices did contain Protected Health Information (PHI) including but not limited to:  patients names, date of births, addresses, social security numbers, medical records, and other information that may be compromised.   We have reported this crime to the authorities, and it is under investigation.  We want you to be aware so you can be proactive and monitor your credit report.  Liv-On Family Care Center, PA is committed to providing quality care and we have also taken additional steps to ensure the security of our clinic.  If you have any questions or concerns, please feel free to contact our Privacy Officer at (800) 317-7320.
